New Hampshire Vital Record and Health Statistics

The New Hampshire Division of Vital Records maintains all the vital records of the state. The certified copies of birth, vital recordsdeath and marriage records that occurred in Concord, New Hampshire are issued by the City Clerk's Office. The office also has statewide records for births starting with 1982, deaths beginning with 1990 and marriages beginning with 1989. Divorce records prior to 1990 are available with the courthouse where the divorce took place.

How to apply

Fees: A $15 fee applies for each copy of a vital record.

Application: Vital Records cannot be requested or returned electronically. Applications are available with the City Clerk's Office. Requested application form must be completed, attached with a valid photo ID card and then mailed to the following address:
